CATHOLIC RITUAL OF BRINGING COMUNION TO THE SICK
May the peace of Christ be with you… And also with you.My brother (s) and/or sister (s), to prepare ourselves to receive the sacred mysteries, let us call to mind our sins.
I Confess to almighty God, and to you, my brothers and sisters, that I have sinned through my own fault, in my thoughts and in my words, in what I have done and in what I have failed to do, and I ask Blessed Mary, ever Virgin and all the Angels and Saints, and you, my brothers and sisters to pray for me to the Lord our God.
May almighty God have mercy on us, forgive us our sins and bring us to everlasting life, Amen.
Let us pray with confidence to the Father in the words our Savior gave us:
Our Father who art in Heaven, hollowed be thy name, thy kingdom come, thy will be done, in earth as it is in heaven. Give us this day our daily bread and forgive us our trespasses as we forgive those who trespass against us; leave us not into temptation and deliver us from evil. Amen.

This is the Lamb of God, who takes away the sins of the world; happy are those who are called to his supper. Lord, I am not worthy to receive you, but only say the word and I shall be healed.
(The Minister gives the Host to the sick saying:)The Body of Christ, Amen
(After a moment of silence, say:)
Let us pray: Father almighty and eternal God, our brother (sister) has received the Eucharist with Faith in you and your healing power. May the body and blood of Christ bring him (her) lasting health of mind and body. We ask this in the name of Jesus Christ Our Lord, Amen.

Yo confieso ante Dios todopoderoso y ante vosotros hermanos que mucho he pecado de pensamiento, palabra, obra y omisión, por mi culpa, por mi culpa, por mi grañidísima culpa. Por tanto ruego a Santa María Virgen, a los Santos apóstoles y a vosotros hermanos, que roguéis por mí a Dios nuestro Señor, Amen.
El Señor todopoderoso tenga misericordia de nosotros, perdone nuestros pecados y nos lleve a la Vida Eterna, Amen.
Oremos con confianza al Padre la oración que el Salvador nos enseñó:
Padre Nuestro que estas en el cielo, santificado sea tu nombre venga a nos tu reino y hágase tu voluntad así en la tierra como en el cielo. Danos hoy nuestro pan de cada día y perdona nuestras ofensas así como nosotros perdonamos a los que nos ofenden, no nos dejes caer en la tentación y líbranos del mal, Amen.
(El Ministro de la Eucaristía toma la Ostia del Pyx, se la enseña al enfermo y dice:)
Este es el cordero de Dios que quita los pecados del mundo, dichosos los invitados a la cena. Señor yo no soy digno de que vengas a mi pero di una sola palabra y mi alma quedara sana.
(El Ministro le da la Ostia al enfermo diciendo:)
El Cuerpo de Cristo, Amen.
(Después de un momento de silencio el Ministro dice:)
Oremos, Padre, todopoderoso y Eterno Dios, nuestro hermano (a) ha recibido la Eucaristía con Fe en ti y en tu poder salvador. Que el cuerpo y la sangre de Cristo le traigan salud duradera al alma y cuerpo, esto te lo pedimos en nombre de Cristo nuestro Señor, Amen.
